Membership Plan Details
Annual Membership & Renewal
The Wilmette Dental Membership Plan is a dental membership plan and is not considered a dental insurance plan. The dental membership plan is for patients without dental insurance and cannot be used in conjunction with a dental insurance plan. Members may only be admitted to the plan upon initial signup or annually thereafter on the renewal date. Membership plan will automatically renew on the 1st or the 15th of the month of their anniversary date unless a written request to cancel is received 30 days prior to the renewal. Annual renewals will be charged at the base annual plan fee plus an additional 3% increase annually.
Use Of Plan & Services Covered
This plan may be used at Wilmette Dental and applies only to procedures offered at Wilmette Dental. This membership plan cannot be used at any other locations or dental offices.
Specialty Dental Needs
There may be dental care needs that require services completed by a specialist. Referred services are not covered by this membership plan.
Membership Dues
The annual membership fee of $599 is due upon sign-up, which can occur on the 1st or the 15th of any month. Dues are subject to change annually; however, an annual increase of 3% is anticipated. Services will not be rendered until the annual due is paid in full; any services completed prior to the annual dues being paid are billed at the full fee.
Cancellation Of Membership
Notification of the cancellation of the plan must be made 30 days prior to the renewal date. There are no refunds for the subscription dues.
Payment For Services
For additional services, a member will receive 10% off the UCR (usual, customary, and reasonable) fee of the practice. Payment for services is due at the time services are rendered.
Scheduling Services
It is the member's responsibility to contact the office to schedule their treatment. For cancellations or no-shows with less than 24 hours' notice, a $75 cancellation fee will be charged. Members must pay this cancellation fee in full prior to be able to reschedule their appointment. If any member cancels an appointment with less than a 24-hour notice three times within their membership year, their membership will be cancelled without a refund. If you do not receive the services within your membership year, any remaining services are void and do not carry over or carry over annually.
